• Arguing counsel who have not previously entered an appearance in the appeal need to submit the Entry of Appearance form.
• For cases submitted on the briefs, use the Custody Status Questionnaire (if applicable).
• For criminal, habeas, immigration or other appeals where the plaintiff, defendant, or petitioner may be in custody, submit the Calendar Acknowledgment Form and the Custody Status Questionnaire.
• For civil cases without parties in custody, submit the Calendar Acknowledgment Form.
